Plasma Renin

Direct Renin Plasma Renin Activity (PRA) Renin
EDTA plasma (standing for ≥2h before sampling; avoid beta-blockers if possible) mIU/L or ng/mL/h

About this test

Enzyme secreted by the renal juxtaglomerular apparatus. Measured alongside aldosterone to calculate the aldosterone:renin ratio (ARR) for primary hyperaldosteronism screening.

Reference Ranges

Sex Age Range Notes
All ≥ 18 yrs 5 – 60 Ambulatory (upright) range (direct renin; highly method-dependent)

Clinical Interpretation

Low / Decreased
Suppressed renin with elevated aldosterone — primary hyperaldosteronism.
High / Increased
Elevated renin — secondary hyperaldosteronism, renal artery stenosis, Bartter's syndrome, dehydration, pregnancy.
